Your Health Benefits

Who's Eligible?

Employees

All regular full-time employees (scheduled 30 hrs/week), eligible on the 1st of the month after 60 days. 

Spouse/Partner

This includes the full-time employee's legal spouse or domestic partner eligible for all health benefits.

Dependents

Children and legal dependents or eligible for coverage up to the age of 26.

JRT Healthcare

JRT is fully self funded in our insurance package. Healthcare Management Administration (HMA), is our 3rd party administrator. At JRT we offer a full benefits program with multiple options for different families' and individuals' needs.

 

Please review the information below to better understand your options.

Employees have a choice between a HDHP plan, mid-plan or a buy-up plan.

How to Enroll?

- You do not have to do anything right now. Review the plan information in the Benefits Guide and and insurance coverages.

 

Call Human Resources if you have any questions.

- Make an appointment with an Enrollment Agent from Colonial Life using the link below. During the appointment the agent will walk you through the enrollment process, explain the benefit package to you and assist you with the enrollment into the plan.

- You can to set up your enrollment during business hours if you need to. Just contact your supervisor or foreman and schedule that in ahead of time.​

If you need to make any changes to your plan, or if you a new employee enrolling into the plan, you can schedule an appointment with an Enrollment Agent who works for Colonial Life.

 

During your appointment, the agent will walk you through the enrollment process and assist you with enrollment into the plan.

 

Your appointment will either be a phone call or zoom style meeting.

Need to change your plan or
did you have a qualifying event?

Click below to see instructions on how to add your baby to the ease enrollment website. 

No 1.

If you experience a qualifying event during the plan year such as marriage, divorce, birth or adoption of a child, or a spouse/domestic partner losing or gaining other coverage, you would qualify to make changes to your benefit plans.

No 2.

To make these changes a new enrollment form indicating the qualifying event and updated enrollment status is required for all benefits within 30 days to be delivered to Human Resources.

Failure to notify Human Resources within 30 days may disqualify any enrollment status changes.
